24-Hour Esophageal pH-Impedance Testing

     24-hour esophageal pH-impedance testing is a technology used to diagnose acid reflux disease. A doctor collects data on acid levels, which is then analyzed to determine the true cause of a patient's symptoms.

 

This test measures the following:

  • The number of reflux episodes.
  • The relationship between symptoms and reflux episodes.
  • Belching from the stomach and upper esophagus.
  • The characteristics of reflux:
    • Physical properties: liquid, gas, or mixed.
    • Chemical properties: acidic or non-acidic.
  • The level of reflux (acid and bolus).
  • The clearance of acid and bolus.

Test Procedures

There are two common methods for this test:

 

  • Catheter: A small catheter is inserted into the esophagus through the nose and collects data for 24 hours.
  • Wireless Capsule: A capsule is attached to the esophageal wall to collect data. The capsule detaches naturally and is passed in the stool. This method only detects acidic reflux.

 

Pre-Test Preparation

To ensure an effective test, patients should prepare as follows:

 

  • Inform your doctor: about any pre-existing conditions, drug allergies, medications you are taking, and past surgeries.
  • Stop smoking and drinking alcohol: at least 7 days before the test.
  • Do not eat or drink: after 6 PM the day before the test.
  • For diabetic patients: Do not take oral or injectable diabetes medication on the day of the test.
  • For high blood pressure patients: Take your medication with half a glass of water at 6 AM on the day of the test and do not drink more water until the test begins.
  • Stop taking the following medications: at least 7 days before the test. If you are unsure, consult your doctor.
    • Acid reducers (e.g., Miracid, Pariet, Nexium).
    • Medications that affect the function of the esophagus and intestines (e.g., Gasmotin, Ganaton).
    • Muscle relaxants.
    • Medications that dilate blood vessels (e.g., ISMO).
